Kona Pediatrics is seeking a pediatrician to join its team of hospitalists at Kona Community Hospital on the Big Island of Hawaii. Responsibilities include newborn nursery, pediatric inpatients, ER consultations and occasional high risk deliveries.
As a community hospital, our pediatric inpatient population generally consists of healthy newborns and children with illnesses of low acuity. There is no formal NICU or PICU so all critically ill patients are transferred to the local children’s hospital which houses all the pediatric sub specialists. Such children are transferred either directly from the Emergency room, or from the nursery after being stabilized.
Average census is 6-7 patients per day; mostly healthy newborns. A typical day begins with 7am rounds on the mother-baby unit. As a level one community hospital, we only care for full term and late pre-term infants, >35weeks gestation. Afterwards, physicians round on any pediatric inpatients. On average there are 1-2 inpatients per day, sometimes none. Common admitting diagnoses include bronchiolitis, gastroenteritis, dehydration, pneumonia and cellulitis. After morning rounds, physicians are considered on call for the remainder of the day’s 24 hours, and are free to leave provided they remain within a 20-30 min radius of the facility. Hospitalists are expected to work ten 24hr shifts per month. These shifts need not be consecutive. Candidates should be comfortable charting within an EMR program. Billing is all done electronically through an app on the physicians smartphone or tablet.
Physicians are expected to respond to ER consults by phone or in person.
All C-sections are attended by the on call pediatrician.
Pediatric hospitalists will attend deliveries at the OB’s request for concerns such as LGA fetuses, meconium, fetal intolerance to labor, or any other maternal issue affecting the fetus.
As KCH is a level one hospital, every effort will be made to transfer mothers in pre-term labor to an appropriate facility. However, sometimes mothers present in advanced labor and need to deliver in house. In these situations, the pediatric hospitalist is expected to resuscitate and stabilize the infant until the transport team can medivac the infant to an appropriate facility.
