Coordinates the medical and rehabilitative services provided for each patient using an interdisciplinary team approach. Coordinates team conference for all patients and facilitates interdisciplinary practice. Acts as a liaison between the patient, family, physician and interdisciplinary team. Facilitates the patient's, family's and caregiver's participation in the development and implementation of the overall plan of care whether the patient is an adolescent, young adult, adult or senior. Collaborates with job share associate.
Basic computer skills
Ability to communicate effectively, both verbally and in writing
Negotiating skills which support the ability to interact with physicians, interdisciplinary team members, discharge planners, external case managers and payers
Minimum of five years of recent clinical experience as an RN.
