General Pediatrician - School-Based Health Center (SBHC), Cohen Children’s Medical Center, Northwell Health
The Department of Pediatrics of Northwell Health and The Zucker School of Medicine seeks a Board Certified/Board Eligible General Pediatrician to join the School-Based Health Center (SBHC) of Cohen Children’s Medical Center at a NYCDOE campus in Jamaica, NY.
The SBHC Site Coordinator/Physician will provide core clinical services and core administrative activities; primary and preventive health care, diagnosis and treatment of medical conditions and management of chronic conditions;
•Age appropriate reproductive health care; •A HEADDSSS and an assessment of risk behaviors; •Immunizations consent, provision and documentation; •Population based assessments as well as responding to individual needs; •Also considered a core service is on-site routine management of chronic conditions (asthma, diabetes, etc.), provided in consultation with the child's outside primary care provider or specialist as appropriate. On-site medications and prescriptions are provided for minor problems, acute problems; and chronic conditions. Laboratory evaluations are available as needed for both reproductive and general medical health needs.
Cohen Children’s Medical Center opened in 1983 as the New York metropolitan area’s only hospital designed exclusively for children.
