The Director Care Management - Fortify is responsible for developing and leading a robust care management program for Fortify, including developing and aligning care management processes and policies across Fortify’s parent organizations, the University of Virginia (UVA) and Children’s Hospital of The King’s Daughters (CHKD). The Director, Care Management reports to the Executive Director of Fortify.
• Works strategically and collaboratively with members of the Fortify Executive Team and Care Management Steering Team to develop a strategic vision, care management roadmap and execution plan to improve attributed patients’ health and wellbeing, enhance service provider outreach and adoption, and successfully grow the program. • Collaborates with leaders of other care/case/disease management programs at UVA and CHKD to ensure a well-integrated and seamless process and system for staff and providers wishing to access care management services for their patients. • Provides oversight of care management duties including screening, outreach, assessment, interventions, gaps in care, and connection with community-based resources. Facilitates development of standardized documentation processes and monitors for the highest performance in service delivery. • Develops, directs, implements, and evaluates new and existing care management service opportunities, prioritizing at-risk populations to enhance patient outcomes. • Influences staff, physicians, and leadership by sharing knowledge regarding the value and appropriate use of this program. • Networks within the parent organizations, network provider groups, and the community to identify resources and information critical to the care managers as they work to achieve the highest quality of care for attributed patients. • Assesses and develops recommendations for new or revised care management policies, procedures, and guidelines. • Monitors and supports quality improvement efforts tied to the care management program. Provides multi-disciplinary case consultation as needed. Collaborates with researchers and other program support to demonstrate value while streamlining the care experience. • Supervises assigned staff to include interviewing, selecting, training, motivating, evaluating, counseling, disciplining and terminating in compliance with EEO/AA goals and personnel policies of the organization. • Ensures compliance with contractual, regulatory, and accreditation requirements.
Position Compensation Range: $96,907.20 - $155,043.20 Annually

MINIMUM REQUIREMENTS • Education: Bachelor's degree in business, healthcare or a related field required. Master's degree in nursing, public health or healthcare administration preferred. • Experience: Minimum five years of program management experience in a related field, diversified healthcare or health insurance company. Minimum three years of experience in a clinical environment (e.g., faculty practice plan, clinical department of an academic medical center, medical group practice, or clinical operations department of a managed care organization) supporting physicians, process improvement and operational initiatives. Experience with Epic, Cerner or similar EMR; or managed care software applications.
