Overview Our Maternal-Fetal Medicine Centers provide innovative, comprehensive and interdisciplinary care for women. As the only Maternal-Fetal specialists in the region associated with an academic medical center, our specialists bring their own expertise as well as collaboration with neonatologists, geneticists and other University of Minnesota Health specialists to provide the highest level of care for women and their babies from preconception to post-partum. We care for women who are: Of advanced maternal age Pregnant with a child who has indications of an abnormality such as spina bifida or Down Syndrome Pregnant with multiples We also care for women who have: A history of preterm delivery A history of preterm labor An incompetent cervix Gestational diabetes History of preeclampsia Pre-existing medical conditions, such as cardiac disease or cystic fibrosis Prior fetal or neonatal death Recurrent pregnancy losses This position will be working on a casual/as needed basis on the day shift, no weekends. The position is not eligible for benefits. The Patient Care Management Coordinator provides comprehensive care coordination of patients as assigned. The care coordinator assesses the patients plan of care and develops, implements, monitors and documents the utilization of resources and progress of the patient through their care, facilitating options and services to meet the patients health care needs. The intensity of care coordination provided is situational and appropriate based on patient need and payer requirements. This position is accountable for the quality of clinical services delivered by both them and others and identifies/resolves barriers which may hinder effective patient care. This position has responsibility to determine how to best accomplish functions within established procedures, consulting with leader on any unusual situations. Internal customers include all levels of nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External customers include physicians and their office staff, payers, community agencies, provider networks, and regulatory agencies.
Required Education:Bachelors Degree in Nursing Experience:5 years clinical experience License/Certification/Registration:Active MN Registered Nurse license,Basic Life Support (BLS) Preferred License/Certification/Registration:Case Management Certification We can recommend jobs specifically for you!
