Summary
To practice in an advanced role to provide developmentally appropriate and age-appropriate Palliative Care services to patients and families both inpatient and in the ambulatory setting with primary responsibilities at Texas Children's Hospital in the Woodlands. The nurse practitioner (NP) works under a delineation of clinical privileges and works collaboratively with other members of the health care team. Practicing under established guidelines and clinical protocols, the NP assumes independent and interdependent responsibilities in the assessment, diagnosis and initiation of treatment and procedures. As an advanced practice nurse, the NP is responsible for education, research, consultation, and evaluation of clinical practice at various levels.
Preferred:
Pediatric or Family Nurse Practitioner with 3-5 years of experience as a nurse practitioner. Strongly Prefer palliative care experience with the ability to provide inpatient/outpatient visits.
Master of Science in Nursing
Advanced Practitioner Registered Nurse license from the Texas Board of Nursing. License may be Pediatric Nurse Practitioner (PNP), Acute Care Pediatric Nurse Practitioner (ACPNP), or Family Nurse Practitioner (FNP).
Specialty certification recognized by the Texas Board of Nursing. Specialty certification must match licensing specialty area. (For example, a Nurse Practitioner with an ACUTE CARE PEDIATRIC NURSE PRACTITIONER (ACPNP) license from the Texas Board of Nursing must also have an ACPNP certification.)
A minimum of three (1) year of RN experience providing care in a hospital, outpatient or hospice setting to chronic, complex patients or patients nearing end-of-life.
Job Purpose
Provides advanced nursing care to patients in a clinical setting in an area of specialization under the direction of a physician.
Job Duties
Obtain detailed history and perform physical examinations.
Perform, order, and interpret diagnostic procedures in accordance with relevant health concerns of the patient.
Prescribes appropriate pharmacological and non-pharmacological treatments.
Maintain records and write orders and progress notes in patient charts as to status, treatments, and procedures.
Monitors and ensures accuracy of recording on patient medical records.
Provide follow-up health maintenance care to patients in accordance with protocols established with the supervising physician.
Communicate with physicians, patients, and the family members of patients concerning patient care.
Collaborates with providers, and practice staff in identifying appropriate patients for care management.
