Description
Nemours Children's Health is seeking Advanced Practice Providers to join our Hematology Oncology Team in Jacksonville, Florida!The incumbent will practice collaboratively with Hematology/Oncology and Blood and Marrow Transplant (BMT) physicians and the interdisciplinary care team to provide high-quality care to pediatric transplant and hematology/oncology patients.

Key Responsibilities
- Performs comprehensive assessment of patients and families during inpatient admissions, outpatient visits, transplant evaluations, and follow-up care.
- Performs ongoing assessment and evaluation of patient status, treatment response, transplant-related complications, and revises plans of care accordingly.
- May use prescriptive authority, if appropriately credentialed, to manage supportive care, symptom management, and treatment-related needs.
- Collaborates with physicians, nurses, pharmacists, social workers, case managers, and other specialists involved in transplant and oncology care.
- Identifies patient and family needs for additional resources and initiates referrals to appropriate services.
- Assists in developing and coordinating individualized plans of care for complex hematology, oncology, and stem cell transplant patients.
- Documents patient care, treatment plans, transplant progress, and clinical outcomes in the medical record.
- Coordinates discharge planning and transitions of care for transplant and oncology patients in collaboration with the multidisciplinary team.
- Provides patient and family education regarding diagnosis, treatment plans, chemotherapy, transplant processes, medications, symptom management, survivorship, and long-term follow-up care.
- Evaluates treatment adherence and patient progress through inpatient management, outpatient visits, and telephone triage as appropriate.
- Acts as a patient and family advocate throughout the continuum of cancer and transplant care.
- Serves as a liaison between families and members of the transplant and hematology/oncology care teams.
- Utilizes advanced clinical knowledge and specialized skills in pediatric hematology, oncology, and stem cell transplantation in accordance with professional practice standards, organizational policies, and applicable regulations.
- Promotes evidence-based standards of care within the Hematology/Oncology and BMT programs.
- Serves as an educational resource for nursing staff, learners, patients, families, and multidisciplinary team members regarding oncology and transplant-related care.
- Collaborates with division leadership and nursing management to monitor and improve quality, patient safety, clinical outcomes, and program performance.
- Participates in quality improvement, research, and evidence-based practice initiatives related to pediatric hematology, oncology, and stem cell transplantation.
- Maintains current clinical knowledge through participation in specialty education, professional organizations, and continuing education activities.
- Demonstrates effective communication skills with patients, families, physicians, and members of the interdisciplinary care team.
- Participates in departmental and enterprise programs for Quality Assessment and Performance Improvement, identifying opportunities to improve patient care, operational efficiency, and clinical outcomes.
- Demonstrates proficiency in caring for pediatric hematology, oncology, immunotherapy, and stem cell transplant patients across the continuum of care, including inpatient and outpatient settings.

Qualifications
- Current certification by the National Commission on Certification of Physician Assistants or current certification as Pediatric Nurse Practitioner
- Florida Licensure as a Physician Assistant required upon hire or Florida Licensure as an Advanced Practice Nurse required upon hire
- Acute or Primary Care Certification acceptable, Pediatric Nurse Practitioner preferred (PNP-PC or PNP-AC) if an Advanced Practice Nurse
- BLS required upon hire from the American Heart Association
- Prior healthcare/patient care experience strongly preferred
- Experience working with Pediatrics or Child Health preferred
- Federal DEA registration either active or required upon hire
